Do you need this for a line diagram or is a report grouped by location acceptable?
This could be easily created by modifying an existing report.

I guess removing a field in the shape sheet would be similar to adding one? I'm trying to do the same thing, but I'd like to add the Location field with the Model and Component ID fields. Any advice would be greatly appreceated.....

You can add and remove any of these fields listed from the shape sheet. I only suggest you keep either of the category and componentid fields. A D-Tools shape is defined as a shape with either prop.Category or prop.ComponentID.
