SP3 prompts for a Description when you create a Revision and tracks the Revision Create date. This is displayed in revision grids and UI. Check out http://support.d-tools.com/001SI5.5_SP3_Release_Notes

In the Reporting center, klick on the expand tree-icon on the project you are working with (same as you do to create a project delta). Then you get a list of all revisions with revision comments.

I was referring to accessing this data programmatically via the reporting API.
