If i add an item with prompted accessories, but hit cancel at the accessory prompt, the item is still added with none of the accessories. This seems counter-intuitive.

i would suggest not adding the item at all if Cancel is selected
OR don't give a cancel option, if the user wants to add without accessories they can "Clear all" and add the item.

I understand what you are referring to, however the dialog box that pops up is for the accessories not for the product (the product has already been added to the project). Therefore if you click "Cancel" you are canceling the dialog box for the accessories.

I guess what I am saying is that for me if I selected "cancel" and the main item was not added, I would see that as not being correct.

I can see this too, i guess the problem is that it's ambiguous. So i would think probably better to remove the "Cancel" button.
