This is how it was explained to me by support...
When you open a project it creates a temporary Access database (.mdb) that stores all of the changes you are making to a project. When you hit "Save" in the project it commits those changes to the project file (.dtl). When you close the project the .mdb file will delete itself. If for some reason the project is not closed properly (visio crashed in mbarnick's case) then the .mdb file is not automatically removed. The next time you go to open the project the application thinks that the file is still in use because there is already a .mdb file.
The way to fix it is to go into your local project folder ("My Documents\D-Tools SI5 Projects" by default) and find the .mdb file for that project and delete it. Keep in mind that when you do this you are deleting the changes that you made to the PDM since your last successful "Save" (see previous paragraph). Unfortunately I was told that there is not a way to recover the changes that you are losing...so SAVE, SAVE and SAVE again while you are working!!!
